Personalised care gives people choice and control over how their care is delivered. Community based adult social care makes the most of existing assets in a community to support all residents and offer preventative support more widely.
In 2025/26 we have retained a contractor to look at the changes that have the biggest impact when adopting a strengths-based approach. Delivering effective community provision can promote local health and wellbeing. We built on work undertaken in 2023/24 (t community coordinated support for the adult social care front door) by testing out our work with several councils.
