Supporting positive transitions is important for all children but in particular children with SEND. This section aims to collate resources which support specifically planned transitions, from a strategic to an operational approach.

This service worked closely with local schools, ensuring that children with SEND were included in mainstream education as early as possible. The program involved both direct work with children and collaborative efforts with parents and schools to ensure each child’s needs were met."
